Patient Portal
The Health and Wellness Center uses an online . All documents and information relating to medical and mental health care, immunizations, and pharmacy needs are housed within the Patient Portal. It is important that all students are familiar with this portal and how to access it. You can use your °ÄÃÅÀÏÆæÈËÂÛ̳ network credentials to access the Patient Portal to enter your information and upload documents and forms as necessary. Please read your enrollment information for more information about your network credentials.
Immunization Requirements
To comply with NYS Public Health Law and meet the St. John °ÄÃÅÀÏÆæÈËÂÛ̳ University immunization requirements, all new incoming students taking at least 6 credits are required to provide °ÄÃÅÀÏÆæÈËÂÛ̳ with proof of immunity to measles, mumps, rubella, and meningococcal meningitis. We ask that you upload your immunization record from your medical provider to the .
Our current medical record is connected to immunization information from the New York State Immunization Portal. If you have lived in NYS since birth, it is likely we have your records. We communicate through the Patient Portal if we are missing information so please be sure you are checking messages from this system.

Providing immunization information is a critical step for enrollment. Submitting information early in the enrollment process will contribute to a more seamless transition to °ÄÃÅÀÏÆæÈËÂÛ̳. To ensure we are prepared to provide you with the best possible care when visiting the Health and Wellness Center, we ask that you enter the following forms and information into the prior to your first visit.
- Health History Form
- Insurance information (required for athletes)
- A physical exam (required for athletes; please see the information you’ve received from Athletics)
- Consent for Treatment and Privacy Acknowledgement
- Wegmans Pharmacy Prescription Delivery Form
